Nicole Florence, MD

Nicole Florence, MD, is a physician at Memorial Physician Services at Koke Mill Medical Center. Dr. Florence completed her undergraduate degree in biology at the University of Illinois at Urbana-Champaign and is a graduate of Southern Illinois University School of Medicine, where she completed a combined residency in internal medicine and pediatrics. Dr. Florence is board certified in Internal Medicine, Pediatrics and Obesity Medicine.

Max Hammer, MD., FACS, FASMBS

Max Hammer, MD, FACS, FASMBS, a surgeon with Springfield Clinic, leads our medical team. Dr. Hammer is widely recognized among his professional peers for his advanced skill in laparoscopic surgical methods and has specialized in minimally invasive bariatric surgical techniques since the 1990s. He is a graduate of Southern Illinois University School of Medicine and is board certified in General Surgery and Obesity Medicine.

Orlando Icaza, Jr., MD, FACS

Orlando Icaza, Jr., MD, FACS, is a general surgeon with Springfield Clinic, specializing in bariatric and laparoscopic surgery. Dr. Icaza is a graduate of the University of Missouri-Columbia School of Medicine, and completed his residency at Southern Illinois University School of Medicine in Springfield. He is board certified by the American Board of Surgery.


Adam Reid, MD

Dr. Adam Reid recently joined the faculty at Southern Illinois University School of Medicine as Assistant Professor of Surgery and a partner of the surgical critical care team with specialized training in minimally invasive surgery and bariatric surgery. Dr. Reid will see patients with the Southern Illinois Trauma surgical team as well as in a general surgery clinic setting.

Prior to joining SIU, he completed fellowships in minimally invasive and bariatric surgery at Wake Forest University Baptist Medical Center as well as surgical critical care at Yale-New Haven Hospital. Dr. Reid earned his medical degree and completed his general surgery residency at SIU School of Medicine.

Dr. Reid is board certified in general surgery and surgical critical care through the American Board of Surgery. He is a member of the American College of Surgeons, Alpha Omega Alpha Medical Honor Society and Society of Critical Care Medicine.


David Sandercock, MD

David Sandercock, MD, is a physician at Memorial Physician Services at Koke Mill Medical Center. Dr. Sandercock received his undergraduate degrees in both biology and chemistry from Quincy College in Quincy, Ill. He is a graduate of Southern Illinois University School of Medicine in Springfield, where he completed a combined residency in internal medicine and pediatrics. He is board certified in internal medicine and pediatrics.



Elizabeth Warner, MD

Elizabeth Warner, MD, is a general surgeon with Springfield Clinic, also specializing in bariatric and laparoscopic surgery. Dr. Warner is a graduate of the University of Texas School of Medicine and is a fellowship-trained minimally invasive general and bariatric surgeon from the University of Washington in Seattle. She is board certified by the American Board of Surgery.
