Urology Nurse Navigator
The Urology nurse navigator is a registered nurse who has many years of experience with urologic patient care. The nurse navigator offers support and personalized services during and after an inpatient stay at Memorial Medical Center. The navigator provides one-on-one education and emotional support for you and your family.
The navigator shares information related to medical, social and community services if you need additional resources. The navigator will advocate on your behalf to your physicians and hospital staff, and communicate with your provider’s office about your hospital stay. When you are preparing to go home, the navigator will arrange a follow-up plan for you before you leave the hospital.
Patient Care Facilitator (PCF)
Your patient care facilitator oversees all aspects of your care upon arrival to your hospital room. These registered nurses monitor your progress daily. They consult with your providers regularly to ensure you are receiving optimal care and your recovery is progressing.
They monitor the quality of your care, including vaccine statuses and hospital protocols for your diagnoses. They can answer questions and coordinate any additional resources you may need before you leave the hospital. They are the gatekeepers of your care, and they work closely with you, your family, and your Urologic Service team.
A social worker will help you and your family solve or cope with problems that have arisen before or during your hospitalization. If you need help with legal documents, social situations or funding for treatments, medication or transportation, they can refer you to many resources within the community.
The discharge planner arranges any home health agencies, nursing home placements, post-hospital rehabilitation, hospice services or any special needs at the time of discharge. They can help make the transition as seamless as possible.
