Patients in Memorial’s heart failure clinic receive education and monitoring that helps them maintain or improve their quality of life. In addition, the providers ensure the right type and amount of medications are ordered to best meet the patient’s individual needs. In addition to Memorial Medical Center, heart failure clinics are also available to patients at Passavant Area Hospital, Abraham Lincoln Memorial Hospital and Taylorville Memorial Hospital.
Some patients are monitored using telescales or the implantable CardioMEMS device. This device transmits wirelessly to a receiver that allows the care team to assess changes in the patient’s body metrics and adjust treatment accordingly.
